We use a table that lists the common stakeholders, and helps policy owners walk through the process of identifying the purpose of the communication/review, etc. However, it is not required. I track the review and approval process from my Policy Advisory Committee point through the President's Policy Committee review, 30-day review, and final publication. The policy owner handles the tracking up to that point (review within their internal group, within their larger management body, key constituents, and faculty governance as appropriate.)
