Cynthia, We don't currently use such a matrix - although I can see how it would help to facilitate the process and ensure proper accountability and increase "buy-in". For the time being we use a flow chart that I found on the ACUPA web site ( and edited for our purposes). We also include a "review process" as a supporting policy development document: Policy Development Procedures, Roles & Definitions.
