i bet you''re putting a transition at the end or
begining of a clip that has no handle. i can't really
explain it very well so i googled it and downloaded
this pdf. it's on pages 132-133.

or you can google about Inadequate Head Handles or
Inadequate Tail Handles.
this problem drove my crazy me for the longest time
until i took a class in PPro and the teacher explained
it to me.
