You need to look a little more closely at your manuals. ASC=3A ASCQ=00,
according to the TSM 5.2 Messages manual, page 985:

        0x3A    0x00    Medium not present

For some reason, the tape did not make it to the point in the drive
where it could be read, even though the library thinks it did. You may
have a hardware issue going on.
