I had WC just stop coverage mid day last fall. We had missed some innocuous piece of mail that had some kind of survey. So rather than call, they just cancelled the policy. I immediately furloughed all the employees. Right at the moment I found out. Took a day to fix. I asked the bureaucrat why cancel mid shift, what does he want to tell my employees? He seemed oblivious, like why would you send everyone home, we will work it out... yeah right.

A workers comp inspector did extensive interviews and visited with the installers. The inspector then assigned the above codes and provided his written documentation of how and why he assigned those. Ask your insurance company to do a classification audit. You can quit guessing, and you have something in writing to point to if the company ever disputes a claim over classification.
