The utilities really seem to have gone away from climbing pegs on wood poles. If ladder doesn't reach, they use a bucket truck. Maybe safety reasons? Also to prevent unauthorized climbing? I can't think the last time I saw a power or phone tech climbing a pole.

Those pegs don't seem well suited to tie off to, and if the one you're standing on rips out of the wood, it's probably going to be a bad day. Unless you install a safety cable.

I found some instructions that say " Installed by placing plate over lag
until plate cuts into pole".  Does that mean you turn the plate by
hand?  What if I cheat and replace the lag with a hex head one so I can
use an impact driver?
