It’s pretty goofy, and it depends on how the end user wants to collect it. The basic version is the feds set a % rate every quarter but it only applies to Interstate phone traffic. If the billing system can keep track of the actual fees charged to the customer for Interstate traffic, or the percentage of Interstate traffic for a flat rated service then the tax only applies to the Interstate traffic. Since that can be a pain in the rear to track the FCC also allows a ‘safe harbor’ number that says 64.9% of voice traffic is Interstate. Reportedly the FCC came up with that number by looking at Vonage traffic years ago, which was obviously heavily weighted to Interstate calls since it was a cheap workaround to toll charges. It’s likely not accurate any more but the FCC isn’t going to change the number.
There are also charges that apply to data lines but I don’t have the details.
