Keepass. It's probably the most widely used *standalone *(not cloud based,
not a service, but something you run yourself) software package for
password management.  Fully open source.

the crypto behind it has been examined to a decent degree of scrutiny.
There are some major ISPs that use it for sensitive things that require
fixed passwords.
