I do not think it matters that the IIS installation has the ashx registered to aspnet_isapi.dll. I believe that IIS can have multiple handlers. As far as the DLL is concerned, it does need to be referenced. When you reference the DLL, Visual Studio will place it in the \bin directory. If you where able to successful "build" the project in Visual Studio the DLL is referenced.
