Recipe: Open a presentation, enable the 3D-Settings toolbar in
 view->Toolbars

Draw a 3D cube from the 3D objects toolbar

Draw a rectangular basic shape and click on the extrude symbol in the 3D 
settings
toolbar.

Now click on the 3D cube and watch the 3D settings toolbar vanish.

Draw a new rectangular basic shape and try to extrude it. You have to reenable
the 3D settings toolbar to get that menu back.

(It comes back if you click on an extruded object, but it goes away for your
to-be-extruded basic shape rectangle so you have to get it via view->Toolbars.)
