Actually, I see that difference fairly often.  Not everybody uses A4 size pages.

a)  Twips, points, and the like are meaningless terms to PHBs.
When they something has to be x mm wide, they mean x, not x plus rounding errors
induced by using twips.

b) Since twips are used internally, why not add twips to the options where
points, mm, etc are used?
