2008 -------
to reproduce:
1. In empty text document enter 3 lines of text (no indent required):
test h1
test h2
test h3
2. Set line 1 style "Header 1", line 2 style "Header 2", line 3 style "Header
3";
3. in menu: Insert - Indexes and Tables - Indexes and Tables, leave default
values (make sure (Create from) "Outline" checked and heading corresponds to
levels in "..."), press ok;
* Normal behaviour: ToC appeared.
4. In "Styles and Formatting" change "Header 1" style: set numbering to any
value, even None (default is <empty>);
5. Press RightMouseButton on ToC, select "Update Index/Table"
* Abnormal: test1, which is "Header 1" style, disappears from ToC and do not
appear there anymore until manually assigned in Tools - Outline Numbering -
Numbering tab (dialog accessible from "Edit Index/Table" too).
