simplier but no so outward reproduction:
1. Create an empty text document
2. Check "Tools - Outline Numbering - Numbering tab": all levels have
corresponding headings; close the dialog;
3. In "Styles and Formatting" change any "Header …" style: set numbering to any
value, even None (default is <empty>);
4. Check "Tools - Outline Numbering - Numbering tab" again: changed header is
no longer there.
