Actually, I've been talking to a colleague of mine today, which experienced
exactly the same issue. He said, after saving and re-opening, there were only
some sort of placeholders in the images places, saying 'xxxx' couldn't be found
or something like that. And I know he's using Windows, so the OS is not
responsible. Nor likely is any of my other actions I performed, while it 
happened. 
While it's true I didn't reproduce the bug, it was obviously by someone else.
And if a bug of that severity (losing all perfectly aligned images is, IMHO)
isn't reproducible on demand, but obviously existing, it's up to the developers
to perform some tests.

Well said. But it's hard to keep cool, if an important standard application
let's you down repeatedly, just when you need it most. And I can assure you that
OO did quite a few times lately. In fact, the 3.x series seems to be so
unreliable that I had to apply to totally bloated backup schedule for every
document (several different versions a day). I just wish I had OO 2.4 back,
which appeared to be rock stable compared to 3.x. I'm starting to doubt there's
any serious testing going on before releasing.

And speaking of bugs, just of today I've learned, that my well-aligned numbering
format in another Document was mysteriously lost 'somehow'. I can't remember
saving it with crapped numbering, but I did actually happen to notice, that all
perfectly formatted titles (up to 3 levels) were flattened to a mere increased
count 1-28.
Now one could say: user error, as it's often claimed here. But as talking to my
colleague showed me, and should show you, bugs DO exist, even if not many take
the effort of reporting them. And I know, I didn't encounter those mentioned
here back in 2.4.

I'm gonna file a new report, but don't expect users to track down every single
bug for you. I don't like to start the discussion about some stone-old bugs or
missing features again, but those sort of are not really qualified to encourage
users to file detailed bug reports, if any. And I'm not even sure, if any
'duplicate' issues are noticed by anyone at all. 
If you're interested in improving your software, you should be glad about any
feedback you get and sometimes initiate some bug-hunting on it. You surely know
your code better than users do and perhaps have an idea, where to start
searching. I can image that you'd like to get more enthusiastic reports about
how nice everything works... but people tend to take it for granted, unless
something doesn't work anymore suddenly. 
Unfair, I know.
