I really liked "A Book on C". I'm not sure if its still in print.

superb book. probably the best programming how-to i've ever read.

True for noteOn, noteOff etc. Not true for MTC, MMC, MIDI Show Control
and other corners of the MIDI universe. Thats why I sprung out the $50
on "The Complete MIDI 1.0 Detailed Specification" from the MMA.
