Hmmm, I guess your best bet is to run

/etc/init.d/alsa start

However, please note that debian doesn't currently use alsa 0.9.X by
default; you probably need to download the package "alsa-source" and
compile it. The simplest way to do that in debian is to install one of the
kernel-source packages and use make-kpkg. The modules will be compiled and
installed (you need to install the new kernel as well!) and the comand
above will probably be run automatically at boot time. Hmmmm... don't know
if alsa 0.9 is only available in the testing distribution...

The CD player doesn't output audio through the sound card, it manages
audio "by itself". Some audio cards have an internal connector that gets
the audio from the drive, and that's why you can hear and control the
volume of the CD through the sound card; but the m-audio doesn't have such
a connector AFAIK.

Yes, try playing a WAV or MP3 file.

Hmmm... all of the card's inputs and outputs. I believe I remember someone
pointing out a small bug regarding this. The ice1712 is capable of
handling 10 channels of audio, but the audiophile only has 2; in spite of
that, alsamixer presents you with controllers for all the 10 channels
supported by the chip.

Hope this helps,
Nelson
