FAILURE AND STRANGE DUMP SUMMARY:
 <client_name>  /vol/vol1/home  lev 2  FAILED [data

 <client_name>  /vol/vol1/home  lev 2  FAILED [dump
to tape failed]

Amanda was performing a backup directly to tape (i.e. no holding disk was involved), and stopped receiving data from the stream for 'dtimeout' length of time. So it killed that particular dump and told you about it.

/--  <client_name> /vol/vol1/home lev 2 FAILED [data
timeout]
sendbackup: start [<client_name>:/vol/vol1/home level
2]
sendbackup: info BACKUP=/bin/gtar
sendbackup: info RECOVER_CMD=/bin/gtar -f... -
sendbackup: info end
\--------

I would like to know what the above messages from
Amanda mean and what may have caused the failure.

For more details on why the dump failed you'll need to look at the debug files in /tmp/amanda on the client and/or in the client's system logs.

Secondly, since I have a limited number of tape media,
I wanted to use the same tape media on which the
backup failed. So as per the "Quickstart" document, I
used the commands

$ amrmtape daily Daily-03
$ amlabel -f daily Daily-03

and again executed the same backup. I would like to
know whether this is the right way of doing things or
I should compulsarily use a new tape media and then
execute the backup.

If that was the only image on that tape, then that's a fine strategy.
