The argument to amavisd-release should specify the complete filename
as reported in the log:
  Blocked SPAM ... quarantine: spam/w/wKa22K6iGE1U.gz ...
                               ^^^^^^^^^^^^^^^^^^^^^^

This should work with $quarantine_subdir_levels set or cleared.
