It doesn't affect sending address, it checks recipient address.
If recipient address is NOT in our domain, we KNOW this is an outgoing mail
(unless we are operating an open relay mailer).

The best approach (as somebody else suggested it already) is to set up
a dedicated local-submission MTA-RX (e.g. listening on a dedicated
IP alias address), which would only accept mail from internal hosts.
Such MTA could then forward mail directly to MTA-TX or directly to 
destinations, without passing it to amavisd. Or it could still pass mail
to amavisd on a separate port, where a policy bank could perhaps
turn off spam checks but still do virus checking, and have perhaps
relaxed banned checks for such internally-originating mail.

Separating externally visible MX from a submission MTA for internal
users has other benefits in flexibility too. I'm not sure how to do that
in sendmail, it is certainly possible, but probably not as elegant as
in Postfix.
