Sounds great. I'm using the default timeout values which is 
1200 secs for timeoutremote and timeoutsmtpd and 60 secs for 
timeoutconnect. Which should be sufficient.

I narrowed it down even more today and this is the error 
message the receiving MTA is telling: "delivery 14: deferral: 
Connected_to_xx.xx.xx.xx_but_connection_died._Possible_duplica
te!_(#4.4.2)".

Is this caused by the buggy client code or something else?

and sent the message; the remote server didn't respond with 
an SMTP code after your server sent "\r\n.\r\n". If it's 
happening to a variety of servers, it might be a network 
problem. A common one is mismatched MTU and broken PMTU -

fragmented but the ICMP must-fragment responses are being 
lost. If you can successfully send small messages but larger 
ones have problems this might be the cause. 

A tcpdump might supply more insight.
