Both sa_userconf_maps and sa_username_maps are mentioned as separate 
changes the RELEASE_NOTES, so I don't think that is the case.  It looks 
like sa_user_config_file itself was completely missed.  It clearly existed 
in pre5:


amavisd:      $sa_user_config_file
amavisd:    my($user_config) = c('sa_user_config_file');
amavisd.conf-default:    ##   $sa_user_config_file

So somehow it got lost between pre5 and pre14.
