In the sample you sent, there were 25 lines of which 21 were in that format,
but 4 were in common format (without the referrer and browser).
You can analyse this logfile by specifying
LOGFORMAT COMBINED # This is the same as the format above
LOGFORMAT COMMON
LOGFILE my.log
It's more efficient though to ensure that your server only logs in one
format.
If that still doesn't fix it, you can turn debugging on to see all the
corrupt lines and what was wrong with them.
