This is likely to be a result of people not correctly setting up the reverse 
DNS on their IP addresses.

A fully qualified domain name, such as www.domain.com, has to have a DNS A 
record to point it to an IP address. An IP address however does not have to 
have a DNS PTR record pointing it to a hostname. (although I'm sure the RFCs 
say it should).

If however you feel that it is somehow your DNS servers fault (I find this 
unlikely), feel free to send me a list of IPs you can't resolve and I'll try 
from here.
