Use the APACHELOGFORMAT command like this:
  APACHELOGFORMAT (%h %l %u %t \"%r\" %s %b %v \"%{Referer}i\"
                   \"%{User-agent}i\"  \"%{Cookie}n\")
(except all on one line).
