Any suggestions as to where I might make the modification to append the path as you described?
Is it something which would need to be set in the configuration file (analog.cfg), the .pl file (anlgform.pl) or the <form> tag in the anlgform.html page?

Internet Explorer tends to ignore MIME types when it finds a known extension,

One trick that helps is appending a path that makes the output look like an
HTML file, as in /cgi-bin/analogform.pl/report.html.
