I found what the problem was: I had this line in the cfg file: HOSTINCLUDE *.*.*.*

When the DNS WRITE was ON, most of the failed
hosts were resolved to e.g. 66-194-6-77.gen.twtelecom.net
so they were listed in the report. The ones that were missing
from the report were the ones without any IP-like data in the
resolved name - due to the implied exclusivity in the INCLUDE
statement. Tricky...

Taking the HOSTINCLUDE line out solved my problem.
