This is actually a good topic, good idea bringing it up.

Just my two cents as usual : It is very hard to make real money out of
ads in your app.

The reason is quite simple, for every 1000 impressions you can earn
from $0.75 to $1 on average.

So you need 1 millions views per month on your app to make $1 000
that's roughly 33 000 ads impression per day.
Compare that with the number of downloads or use(flurry ?) of your app
and it'll give you a good idea of what to expect.

Now money is money, and even if you get a fraction of that, it's a
always nice. But you have to think how it can put off some of your
base users.

Anyway to answer your original questions :

I use AdMob which has a very very low cpm and I am going to look into
greystripes which offers full screen ads.

Using different ad providers in your app, if it's done with care and
it doesn't spoil your design, I would say go for it.
I'm actually working on a webservice(for my use only sorry) to be able
to configure the display of ads or not and their type to do just that.

I put my ads only on menus, not during gameplay. And I try to make it
as non-obstrusive as possible.
I saw a lot of apps who purposedly put the add very close to buttons
to try to get the miss-clicks, i'm sure it is efficient in terms of
money, just very bad in terms of usability and user retention.
