The legal & logistical issues are of course why this is not just a technical issue, but after 2 years? There are dozens upon dozens of digital distribution platforms out there - some of them started after the Android market - that manage to handle purchase and developer sales from multiple countries. It may not be trivial issue, but it is not rocket science either. The only explanation that really makes sense is that Google just isn't interested (i.e., to prioritize it).
I'll second your suggestion to list at alternative markets when possible. The Android market only holds the position it has because it is the default market on Android and thus holds a monopoly on the users of this "open" system. It seems like the only way that one will be able to get Google to pay attention to the deficiencies of their market model is for the alternatives to become big enough that they actually provide some competition.
