The "normal + button" sounds like HTC Sense's home screen, or possibly some third-party home screen that adopted the same look and feel. My guess is that the home screen had a hiccup and missed the new-package broadcast. A reboot of the device, or possibly killing off the home screen some other way, would then restore the "missing" icon.
