Actually with MIDP you could do this - each time an app wanted to
access a file/network you had to give it permission. Of course, the
"don't ask again" option was disabled unless you paid lots of money to
get your app signed.

I agree with this, but I also think it should be hard for an app to
*know* whether permission has been denied or not. Otherwise they will
just check them all at the start and show a dialog saying "You need to
enable all permissions to use this app" and we're back to square one.

It is possible to block app's internet use (very useful!) using an app
called DroidWall, but you have to be root & have a netfilter-enabled
kernel.
