I set up a similar set of pages at http://zmbh.com/discussion/adps/

Visit that directory, download the files include.adp and included.adp
and run. Tell us what happens when you visit include.adp. You might look
at your server.log file to see if any errors are reported. In adps,
errors are usually silently ignored.
