I have had this happen to me once or twice in the past month. Even if the task was already marked completed on the iPhone, after the sync to iCal the task would re-appear as active on the iPhone and show as "overdue". The task would still show up in the completed section on the iPhone. I just cleaned it up manually when it happened. It doesn't happen all the time, as I said just once or twice.
