I'm looking forward to your new approach.  I've come up with a semi-
workable approach that is at least predictable, but not very useful.

Making sure my MacBook is up and running, I bring up iCal.  (I've
stopped updating To Do items when the Mac is not on, as they usually
get reset.) I update just one To Do item on my iPod Touch, then hit
the sync button, and then watch to see if the item changes on my Mac.
It usually does not on the first sync.  At this point the iPod and Mac
are not in agreement with this one task, so I hit the sync button
again.  This second time results in the To Do item getting set back to
what is on the Mac in iCal.  So, I then go back into this one task on
the iPod, make my change, and hit sync again.  This third time is
almost always a charm, and the To Do in iCal will quickly change to
agree with the iPod, and then I'm good to go.  I can now update all my
To Dos on my iPod, and they are immediately reflected in iCal for
basically the rest of the day.  I can also update ToDos in iCal, and
these will sync back to the iPod in one or two syncs.  I have turned
on auto sync to reduce the number of times I need to manually sync.

In addition to being ridiculously convoluted, I am prevented from
reliably updating ToDo items when out and about with only the iPod
Touch, as they will most likely get reset to the iCal contents on my
first or second sync.  Adding new items on the iPod work fine,
however.
