please note also that spamassassin "learns" only via its bayesian
engine. But even if a mail gets 99% probability of being spam, this is
not enough alone to mark it as spam (this gives 3.5 points but by
default you need at least 5). 

It happens to me all the time, I mark mails as spam but they still pass
to my inbox. The solution would be either to give higher score to each
email having 99% bayesian probability, or lower you spam threshold from 5 downto
3.5.
