Thanks for checking Ryan. The name and reference card are set to visible. Permissions appear to be correct. I am the only person doing data entry as well as configuring Arches and I have full admin permissions. Since upgrading to 4.3 and 4.3.1 and restoring back to when the problem occurred with the duplicate Name cards I have created several test records without the error occurring again, although Name Type in the Name card isn't visible during data entry. I will do the same for this graph as I did for the two HIP graphs that had the same problem - delete the two name branches and reconfigure. I'll leave the developer console running to catch any potential future problems.
