you are right, but you forgot to mention that lacnic also pointed out
the all those cases were solved thanks to coordination among ISPs.
All except one, yes.
But the number of cases per year is really low. I guess due to the
desincentive people face when wanting to file a report about it...
no policy or "punishment" was needed ;-)
I hope that at least other regions can have what LACNIC has -- and the
word about its existence can be spread further.
However, the policy gap (in the 5 regions) is still helping those who
decide to engage in continued hijacking.
