I must have missed some setting.  I don't believe it is ARS-specific

Mid-Tier 6.3
Windows Server 2003
IIS 6 (anonymous access disabled, Integrated Windows Authentication
enabled, Require Secure Channel (SSL) enabled, Allow ASP pages enabled)
