1. You will need to user the user tool to create a report. With the form open that you want to print the details about select Tools --> Reporting
Double click the New Style line which should be the first line. This should bring up all the field on the form. Add all field that you want in the report and save the report default type (AR System). You will then open the 'Report' form in User and find the report that currently is being run when you click print case and replace it with the new one that you created. This should solve your issue.
