Basically it's this Most soft drinks are made with corn sweeteners (high fructose corn syrup). Dr. Pepper was originally made using pure cane sugar. There is still one bottling plant in the US that makes the Dr. Pepper using only pure cane sugar. That's what we call the "Real" Dr. Pepper. The plant is in a little town outside Dallas/Fort Worth Texas. It only makes bottles of Dr. Pepper (no cans). The bottles clearly state on them "Pure Cane Sugar".
Everyone I know that has tried them both say there is a big difference between the flavors. I say that the "Real" one is smoother, more pleasant to drink. I can also get them at the steak house down the street from the office "Love and War in Texas". You really know it's a slow day on the list when the discussion on here is about Dr. Pepper.
