** I am trying to get the form name used in Remedy.  I used : SELECT name FROM arschema where schemaid =  'T907' but it is giving me an error message.  I don't have access to the SQL Server.

If you are asking about the name used in a SQL Server query open up SQL Server Enterprise Manager or SQL Query Analyzer and look at the SQL Views.  For each data form there is a SQL View.  For the most part the SQL name of a form is the same as the Remedy name with certain characters substituted.
