If you are asking about the name used in a SQL Server query open up SQL Server Enterprise Manager or SQL Query Analyzer and look at the SQL Views.  For each data form there is a SQL View.  For the most part the SQL name of a form is the same as the Remedy name with certain characters substituted. 
 
See Database Reference Guide v6.3, page 41 for more details.

I have a table name on a set field action SQL statement and I want to know what the form name could be.  Is there anyway to find out a form name using the table name?
