I am running this as a Set Fields action in an Active Link.
That is how I am capturing the return code:
$PROCESS$ PERFORM-ACTION-ADD-ATTACHMENT 536900004
I am setting the result into an Int. field and checking the
result in a later Active Link.

You are specifying the Field ID without the $ ?
i.e. PERFORM-ACTION-ADD-ATTACHMENT 536870913
