I'm not sure if this is the same issue, but I had similar symptoms
when my attachment pool was on a tab that the user didn't have access
to.

I use an Admin tab (Admin only permissions on the tab) to hold all of
my hidden fields, and as long as the user has permissions to the individual fields, everything works fine. Except for the attachment pool for some reason. I can't remember if there was a bug listed or not, but as soon as I moved the attachment pool off the page holder/tab, everything worked fine.

My case is exactly as described... my pool was on an "admin only"
tab. I did as suggested above, and things are now working as expected
for everyone, regardless of group membership.
