You will also notice in your mid-tier logs that it logs the user as using IPv6 formatted address in the first session and then if the user logs from the same machine again it will log the Mid-Tier IP address as the user address.
If that is the case, then "user being locked out" issue has been fixed in MT 7.01. BTW going to this version also resolved some other issues we had so I would suggest that you try it out.
