I'm not sure if I'm understanding the problem correctly, but here's two 
issues I had with requester.

An incorrect Tier 1-3 combination or you've made some fields in helpdesk 
required that out of the box weren't.  Check the values in SRM:Request and 
try pluging them in manually into hpd:helpdesk (incident) to see if you're 
able to submit it.

I have attached menu in summary filed , but the configuration of the 
assignment base on operation categorization.
