Well one thing to check maybe (because I always forget this one) - did you flush cache on the mid-tier BEFORE you tested and after you added the Submit button button, and also flush cache in your browser?

If I log into the client as a user with permissions only to that group and a floating license, everything works fine- the information is submitted and a record created. If I log into the web client as the same user and try it, it does not save. Other workflow on the form runs. The people who will be using this form will be doing it only via the web, so it's important to get working. Why would the Submit work on the client, but not the web? Am I overlooking something obvious?
