Yes I have. Basically, the form selection is where the entry will reside. The entry point active link is the AL that is fired. It is important to note that by setting the entry point AL, it will override the list of active links that you can set on the second page when creating the guide.
Usually if I need additional workflow to fire, I'll setup two guides: One that is assigned the entry point active link, which in turn, calls another active link guide.
