Hi, Did u check into the API logs? I have seen a behaviour in Remedy 7.0 where everything goes on fine (workflow processing) and a request ID is created, but in the end there will be an API error in the end and the whole transaction is rolled back. After this system automatically redoes everything (workflow processing) and a new Request ID is created. So if you see from the front end, everything went on smoothly but the only problem you would see is a Request ID is skipped.
