I believe we met at MIRUG.

If you remember -- Kinetic Request could provide this functionality for you.
It is not Mid-Tier, however.
(I mention this - as it may be way easier to install Kinetic Request on the
server -- than to upgrade 1,000s of PCs with IE 5.01)
(Assuming these are custom PCs at Borders store locations)

The Kinetic Request product allows you to build web interfaces on top of
your Remedy system (ARS 6.0 and higher). The interfaces can be used to
lookup information (like your example above) -- and/or create information
(work orders etc...)

Another approach is to write your own php/java/perl/whatever -- and generate
very simple HTML which IE 5 can digest.
