My issue with Patch 4 was :
a) Requencing of tasks
b) Deleting an alternate approver

They both crashed the server, and I tracked it down to the execution of the
$PROCESS$
Application-Query-Delete-Entry

I installed Patch 5 and tested and the issue went away. I haven't seen
anything funny so far....
Oracle 10g
Solaris 10
